Your USB card might be bad, but a more likely suspect would be how IRIX handles the connection of previously unused I/O devices. When you change from a PS/2 to USB keyboard/mouse, ioconfig adds the new USB kb&m to its configuration list but leaves the PS/2 kb&m active. IRIX expects the sysadmin will examine ioconfig.conf and reorder/reinstate the list as necessary. If you're not running mission critical big iron, the quick and dirty fix is to delete ioconfig.conf and reboot.

If you haven't already done so, you might try deleting or renaming /etc/ioconfig.conf, then reboot the system with the USB kb&m attached. IRIX will regenerate ioconfig with active entries for your USB kb&m (run man ioconfig at a command prompt for some additional detail).

I prefer to save the existing copy of ioconfig, and usually rename to ioconfig.conf.O. That's a capital O (as in Old), which would make the saved copy appear if you ran the versions changed command).
