o2 FPA adapter fail

Please help: I cannot get video output from SGI o2 flat panel adapter. I connect 1600sw via multilink to VGA, I see boot screen OK. Then I connect 1600sw directly to FPA, and no video output (orange blinking led on monitor). I disconnect keyboard, connect serial port and via serial do 'hinv' command from Monitor—no FPA adapter in list. What should I check, should it be on list, is my FPA adapter dead? If yes, then how to fix, any ideas?

Remote flat panel adapter, take toothbrush with rubbing alcohol and scrub both the board and the slot it goes into. Use a hairdryer on low heat for about 10 mins per board, then reassemble and try again.

I have tried. Did not help. Is it possible that problem is in monitor, not in the adapter? Monitor works with MultiLink but is it sufficient sign that the monitor is working absolutely correct?

I've upgraded PROM version. Now works OK.
Also, hinv in Monitor does not show the adapter. It shows only in IRIX.
